Under Reports, I am getting Blanks rather than user names & Unknowns

When I download the report or even look at it on screen, I am getting blanks, sometimes there is even as associated Score, and group-name.

Also had a name come up as a 1st name, in a designated group??
I have no one named "April George, paralegal"  this is all int the first name column???

I tried logging out and starting again, but it continues to appear.

Also why so many duplicated names?  I understand multiple attemps but is there any way to not see the same name appear 5 times on the list??
