Quiz answers "reshuffling" on review?!

Hi, I have had two seperate reports of people who have taken the quizzes in my courses saying:

1) When they review the quiz and look at a question that was answered incorrectly, the questions have been reshuffled, and the "correct" answer is actually wrong.

2)  There was a case with one question - that was NOT shuffled - not accepting the correct answer (sayign the correct answer was wrong) and it actually DID shuffle the questions.  I double checked this in my original file and verified shuffle was turned off and the correct answer was marked.

Anyone else having quirky things like this happening in their quizzes?  I have been unable to duplicate any of these issues on my end, but the two different users SWEAR they have tested the error repeatedly.
