Problem with Integrated Telephony

We have started experiencing a problem with integrated telephony:

After the host opens the meeting room and starts the audio conference, the audio connection window opens a second time.  The only way that the host can close this window is to select the listen only function.   The audio works, but the phone icons next to each person’s name in the Attendance pod are disabled and do not indicate the participant’s status.

We have two hosts assigned to most meetings, but this has never been a problem before.  The problem occurs even when a single host has logged in and started the meeting room.
