Request forms for content uploading

Hello,
Has anyone developed request forms for publishing content that they can share.
Due to the limited authoring licenses purchased, one person is assigned the task of publishing content.

Hi Donna,

I hope its not too late, but I saw this request and then worked with the Adobe internal training team to create this form:

You'll need Adobe LiveCycle Designer, which is included with Adobe Acrobat, to modify the form.
