Instructor Entered Grades - Grade Book

I have a customer who needs to enter grades on multiple items (0-4 + NA) and notes on each task graded.

Task A - 3 - Good control, efficient
Task B - 4 - missing communication, but otherwise excellent
Task C - 2 - was not able to keep it in line all the time

The instructor would enter the numbers and notes to the right.

Questions:
1) Anyone build this type of grade book functionality?
2) If someone were to build it, would it be useful to you or some of your customers?
